We would really like it if you did not send paperwork or forms to the patient when the reminder calls go out. We have seen that patients when getting the reminder they get all the paperwork to sign. The only patients that would need any of this would be NEW PATIENTS. We would rather them do the papers that we have per Dr. Chung's request since program does not have everything in it that we need.
